A rectangular relief plaque featuring the bust portraits of four World War II chaplains is mounted on a stone base with a curved top edge. Each man is depicted in uniform wearing a military cap. Two men are shown in each row. Depicted are: Lieutenant G. L. Fox; Lieutenant J. P. Washington; Lieutenant A. D. Goode; and Lieutenant C.V. Poliq. The busts are encircled by a laurel wreath. Religious symbols, including a cross, appear on either side of the wreath.

The relief plaque honors four clergy members who gave their lives during World War II when the vessel they were serving aboard, the S.S. Dorchester, was torpedoed on February 3, 1943. The four men gave up their life jackets to save four soldiers, then linked arms, lifted their heads in prayer, and went down with the ship. The plaque was installed by Botts Fiarito Post No. 5 of the American Legion on Nov. 11, 1965. The contractor for the project was the Derrick Monument Company of Le Roy.
