Description:
Plaque featuring a relief bust of Lieutenant Colonel Melancthon Smith in uniform and wearing a beard. Beneath the relief is an inscription. The plaque is mounted on the upper front of a two-sectioned base consisting of a stele atop a pedestal.

Remarks:
EC Crummer contributed $80 and Smith's widow, Mrs. Annie M. Chetlain, contributed $100. The base cost $185. Lieutenant Colonel Melancthon Smith served in the 45th Illinois Infantry; he was mortally wounded on June 25, 1863, during the fight in the crater. The monument is on the National Park Service List of Classified Structures (LCS no. 03440). IAS files contain a copy of the LCS report form.
