(Stele, north face, below Connecticut state seal, incised lettering:) ERECTED 1903 BY THE/STATE OF CONNECTICUT AND/THE COLLINSVILLE CEMETERY/ASSOCIATION IN MEMORY OF/THE MEN OF CANTON WHO/OFFERED UP THEIR LIVES A/SACRIFICE IN THE CIVIL WAR/1861 -- 1865./AND WHOSE BODIES/WERE NEVER BROUGHT HOME (Stele, plaque on south face, in raised lettering: List of 39 names, two lines for each name, giving unit, date, and sometimes place of death, and often circumstances) unsigned

The monument consists of a roughly-finished granite stele with a carved image of the Connecticut state seal on one side and a bronze inscription plaque on the other side. The top of the bronze plaque is rounded and contains a laurel wreath surrounded by a band of stars. The stele rests on a pyramid-shaped base.

Formerly located Collinsville Cemetery, Collinsville, Connecticut

Sculpture was erected by the State of Connecticut and the Collinsville Cemetery Association in memory of the Civil War soldiers of Canton, Connecticut who died in the war and whose bodies were never returned home. The monument was dedicated on Memorial Day 1903. When the monument was installed, the cemetery was named the Collinsville Cemetery.
